VTC Request Form

Accessing the VTC App

In order to begin using the VTC system, you must first be given access to the system by an Administrator and provided with a login. To get access, you are required to fill out and sign our Security Access Form and send it to our VTC Statewide Coordinator. After your Security Access Form is received and approved by the VTC Statewide Coordinator, you will be added to the system as a User. When an Administrator puts you into the system, you will receive an email notification with your user name and password.

Download our Security Access Form here.

Also, download our Acceptable Use Policy and Agreement (AUPA) required by the Department for Aging and Rehabilitative Services here.

In accordance with the Acceptable Use Policy and Agreement (AUPA) required by the Department for Aging and Rehabilitative Services, the Security Awareness Training is available for download here.

When you finish filling out and signing both documents, we have given you two ways of submitting them:

  • You can submit it via US Mail to:
Department for Aging and Rehabilitative Services (DARS)
Attn: Joyce Haskins-McKune - VTC Statewide Coordinator
    8004 Franklin Farms Drive
    Henrico, VA 23229
  • Or, if you can scan them to separate files and save them on your computer, you can attach them to an email and send it to:
    VTC@dars.virginia.gov

Until You Receive Access...

After you have chosen the Date, Time, and Site(s) that you would like to request for video tele-conferencing, you'll need to put that information in an e-mail and send it to our team at VTC@dars.virginia.gov. We will add your request to the VTC schedule and send you an email confirmation.
